Directions: Read and Study the CHA ÒMadrid and BarcelonaÓ Itinerary, CHA's Important Information Sheets, CHA's Tour Enrollment Form, CHA Release Form, and the DanÕs Supplementary Agreement Form.

 

We will turn in our enrollment packet for all participants to CHA before Nov 1.  In so doing, we will benefit by a Òfall saverÓ discount of $50.  The base cost of the trip becomes the published cost from Cincinnati at 35 participants = $2079 - $50 Òfall saver discountÓ = $2029.  If you are going, it is critical that you sign up, and make a deposit, or full payment on or before our second meeting on Friday Oct 15. With a larger busload, the cost decreases; with a smaller busload, the cost increases as follows:

If the deposit is made after Nov 1, the base cost respectively is each $50 more at each level.

 

ThereforeÉÉÉ.if you are not sure you are going, DO NOT make a deposit, as this is one unnecessary way which would increase trip cost for the group later before departure, if you drop out.

   3.  Sign and date DanÕs Supplementary Agreement for Group Participation form  (special extra form).

Amount Options:

 

a.     Deposit:  $95 (non-refundable registration fee on or before (Oct 15, 2010). The appropriate balance below applies:

 

+ Balance of base cost + $70(weekend departure and arrival differential--$35 in each direction)+ $537 (domestic departure airport taxes/ airline assessed security/insurance fee, foreign departure airport taxes,  + fuel surcharge; This may increase slightly in January 2011) + any applicable adult and/or room charges + any applicable insurance charges + any share of unfilled room charges by 110 days prior to departure.

 

NOTE THAT TECHNICALLY, ANOTHER DEPOSIT OF $400 MUST BE RECEIVED FROM YOU POSTMARKED NO LATER THAN 30 DAYS AFTER YOUR $95 NON-REFUNDABLE DEPOSIT AND APPLICATION ARE POSTMARKED.  THIS WILL BE NO EARLIER THAN OCT 16TH.  THEN, 30 DAYS AFTER THIS IS ACTUALLY BEYOND OUR ABSOLUTE DEADLINE OF NOV 15th.  THE MAIL AND BILLING OFTEN LAGS BEHIND. FOR THIS REASON, YOUR REAL OPTIONS ARE THE $95 DEPOSIT, FOLLOWED BY THE COMPLETE BALANCE PAID BY YOU TO CHA NO LATER THAN NOV 15th.

b.     Full Payment (I WILL NOT KNOW THIS FIGURE UNTIL OCT 8 AND I SEE THE NUMBER OF APPLICATIONS):

CHA FOLLOWS THEIR OWN PAYMENT POLICIES, THEY ARE NOT FLEXIBLE BEYOND THEM, SO READ THE ENROLLMENT BOOKLET CAREFULLY.

MoreÉread carefully below for other possible applicable charges to full payment:

 

c.     If 22 years of age or older during trip, you pay the ÒRequired Adult feeÓ, add  ($38 x8 days) = $304. You automatically receive a double room.

 

d.    If 22, 23,or 24 years of age, and a full-time college student (must send in copy of college ID with payment), and agree to be in a triple room, your ÒRequired Adult feeÓ is reduced to ($15 x 8 days) = $120.

 

e.     If 22, 23, or 24 years of age, and a full-time college student (must send in copy of college ID with payment), and wishing a double room, you add ($20 x 6 nights) = $120 to the above $120 or a grand total of $240.

 

f.      If under 22 years of age and wishing a double room add ($20 x 6 nights) = $120.

 

g.     If any participant wishes a single room add ($58 x 6 nights) = $348 to appropriate option c, or d.  

 

h.    If optional insurance is desired, payment of $109 on Oct 15, 2010.

 

 

NOTE WITHIN CHA PAYMENT INFORMATION, ALL REMAINING BALANCE IS DUE 110 DAYS PRIOR TO DEPARTURE.  THE DUE DATE IS NOV 15, 2009. 

 

 

Beyond the above, make sure your passport is valid.  In passport language, this usually means that it will not expire until 6 months after departure from the country visited. If your passport is going to expire anytime within (6) months after your expected departure from this trip, if it were I, I would renew a s a p.  I would also pay for the expedited mailing option.  As you may know, recently passports have taken as much as (6) months for conventional submission for renewal.

Supplementary Agreement for Group Participation

 

      This is the fifteenth trip in overseas group travel that I have undertaken, involving more than 200 different participants, over 30 years.  Great excitement, economy, and enjoyment of sharing discovery are associated with this travel. About 50% of travelers on these CHA trips have participated on previous trips. About 20% of travelers have traveled with me three or more times. It is this experience that leads me to make sure that you understand responsibilities associated with the pleasure and convenience of CHA group travel.

IMPORTANT NOTE: CHA IS THE OFFICIAL SOURCE OF INFORMATION!

 

I understand that the only official information is in CHA publications.

 

 

 

In participating on the Cultural Heritage Alliance sponsored trip, I agree to the following:

 

1.  I have read the CHA tour description/itinerary and especially have examined and understand the CHA Important Information, CHA Tour Enrollment/CHA Release Forms.

 

2.  I understand that the $95 Membership and Registration Fee is non-refundable.  I understand that the $95 deposit alone, or with full-payment is necessary to be paid by Oct 8, to receive the Fall Saver base prices listed.

 

3.  I understand that CHA, and neither the College of Mount St. Joseph, nor the MSJ Art Department, nor Daniel Mader is operator and sponsor of the trip.

 

4.  I understand that it is my personal responsibility to have a valid passport/valid to participate on this trip.

 

5.  I understand and agree to contribute my assigned share to the tip for both the European Tour Director and Coach Driver.  This is usually  $8/day per participant per day ($5 for the tour director + $3 for the coach driver) --$56 total for the two. If we have a smaller group, you may wish to consider giving a bit more.

 

6.  I understand that the size of the tour group is limited by coach seating (max. of approximately 48 passengers).  Accepted applicants will be determined by a combination of factors: date of expressed interest, date of received completed application and payment, plus space available in participant category--present student, past participant, newcomer.  The October enrollment will determine the Ògo/no goÓ status of the trip.

 

7.  I understand that this trip has a variable base cost, and that this trip could increase in cost. (For example, if we have 35 paying participants, all paid before Nov 1, the base cost for the trip per person will be a base of $2029 each.  If we would have 3 paying participants drop out after Nov 1, and the number of paying participants is now 32, the cost for the trip would increase to $2104 each. You would be billed for the balance of $75). I agree to pay the cost of the trip based on enrollment of paying participants down to 15 participants.

 

8.   I agree to pay my assigned share of any additional rooming supplement assessed to the group for unfilled rooms.  This is in addition to any double, or single supplement that an individual may already be paying.  The amount is shared by the whole group and collected by CHA about 90 days prior to departure.  The worse case should be a single of each sex left from unfilled triple or double rooms.  For a group of 35, this would be: two singles at $348 each (6 nights x $58) x 2 (male and female) = $696 divided by 35= $19.89 each.

 

9.  I understand that it is my personal responsibility to procure, and maintain my passport.  I understand that I must maintain both my passport, and my air ticket throughout the trip, and to complete a personal information form for Prof. Mader and the Trip Director. (This information is to be available in case of an emergency.)

 

10. I understand that flexibility of temperament is critical to enjoy group travel, and that because there may be some adjustment to destinations--or a different destination altogether, I understand that my interest in group travel must be slightly higher than my interest in a specific destination.

Reminder to all participants: You must have read and studied the CHA Tour Enrollment Booklet 2011, including:  CHA Release & Agreement Form, CHA Tour Enrollment form.  You need to have thoroughly read the ÒMadrid and BarcelonaÓ itinerary, and the Supplementary Agreement for Group Participation form. You must have completed filling in information on the CHA Tour Enrollment form, plus signed the CHA Release & Agreement form (reverse of the Enrollment form), and signed the Supplementary Agreement for Group Participation Form.

 

These two signed forms, plus check must be turned in to me today. 

 

If today, after I collect all application packets, I have more interested applicants than there are spaces available on the trip, I will determine the participant list for this trip, as I have previously indicated.  It will be by a combination of factors: date of expressed interest, date of received completed application and payment, plus space available in participant category—present student, past participant, newcomer.

If necessary, in about a week, I will notify the applicants unable to be accommodated on this trip and return application materials and check(s) to them in the mail.  Unless told otherwise, I will keep them on a waiting list, and call them if space later becomes available.

For those who are not so notified, you will be a participant on the trip and the following information is for you.

 

Regarding payments to CHA, I mail ONLY the initial enrollment materials--CHA Tour Enrollment form/Release form, and check to CHA as a group.  You will be billed individually by CHA for the balance, which you mail in directly to CHA.  If no bill arrives from CHA to you, you are still responsible to make complete full payment to CHA 110 days prior to departure—before Nov 15, 2010.  (See ÒEnrollments and PaymentsÓ section of CHA Tour Enrollment Booklet 2011.)

Please mail earlier than CHAÕs deadlines, and use a US Post Office return receipt method as they suggest. CHA is not flexible on these points, as they must have a policy.  They have as many as 20,000 application packets coming to their offices at this time!!
